Jeffrey Simmons Memorial Scholarship

On August 12, 2005 a tragic accident took the life of Jeffrey Simmons. Jeff was a 1998 graduate of Pisgah High School and an avid athlete. Jeff’s family and friends established the Jeffrey Simmons Memorial Scholarship in the summer of 2006. This is a one-time $1,500 scholarship. The number of scholarships awarded will be based on available funds from donations and the Annual Jeff Simmons Memorial Golf Tournament.

ELIGIBILITY CRITERIA
 Graduating High School Senior from Pisgah
 Scholastic Achievement – minimum 3.2 GPA
 Must be participating in athletics – preference given to those involved in baseball and/ or Canton youth basketball
 Involved in extracurricular activities and the community
 Financial Need
 Write a response on a separate sheet, typed, double spaced, one page in length. Please explain how your involvement in athletics has impacted your life and how you see it affecting your future.

PROCESS
The family of Jeff Simmons will appoint a committee to review the applications. The committee will be review the applications and will make a recommendation for the recipient of the scholarship. The selection process shall consist of a review and evaluation of the scholarship application and possibly a personal interview. The Board of Directors of the Foundation will make the final decision on the recipient of this scholarship. The Board’s decision shall be final. The Haywood County Schools Foundation, Inc., a tax-exempt charitable foundation, will administer the scholarship fund.

PAYMENT OF AWARDS
Payment of scholarship awards will be made at the beginning of the school year or at the beginning of each semester. Checks will be made payable to the school for student’s account and will be forwarded to the financial aid office. A copy of the previous semester
